"THE ONLY THING WORSE THAN GETTING AN STI IS…"

Hi, I’m Andy Kogelnik, MD, PhD a physician-scientist with deep expertise in infectious diseases and sexual health….and this story might change your life.

Curious?

One of my patients—let’s call her Dana for privacy reasons—had the exact experience you are trying to avoid right now.

I tested positive for herpes. It was a swab test because I had a lesion. It was so upsetting and so stressful. On top of that, the herpes HURT SO BAD. It felt like my labia were on fire. I couldn’t even wear underwear. And I felt sick like I had the flu for ten days.

Yeah, it healed up, but it feels like a life sentence. Because IT IS. I can manage outbreaks by taking antivirals. But I don’t want to have to take a pill every day for the rest of my life.

Worse, I read that studies show herpes is linked to cognitive dysfunction, neuropathy and Alzheimer’s.*

I can’t believe this happened to ME! I have always been so diligent and used protection.

I am really, really pissed I got herpes because I always asked people, “When was your last test and what was the status?”

I’m kicking myself for two reasons.

First, I never asked to SEE their tests. Second, I didn’t know people in my swinger community weren’t testing for herpes.

I was testing for HSV 1&2. But they weren’t. They aren’t even doing a comprehensive panel of tests!

I have outed myself to my entire swinger community because we need to do a better job on our testing. I lost a lot of long standing connections, which has been awful. And now it feels like people are scared to touch me or speak to me.

I’m mortified.

I’m trying not to take it personally. I know a lot of this social outcasting from the group is due to their lack of education and knowledge about STI testing.

So I made the decision to become part of the testing leadership, because I am well known within the community. However, on a personal level IDK how to do this.

Doc, can you help me regain my community’s trust and tell us how to truly protect ourselves?

We’ve been playing a game that is a cross between Russian roulette and “safe sex theater.”

Here’s How To Protect Yourself And Your Community

My heart goes out to Dana, her community and all the groups out there trying to navigate the STI landscape. It’s what keeps me working hard every day here at ProDx to spread the word (and not the herpes, syphilis, gonorrhea, etc. etc. etc.) about how great at-home STI collection kits are.

It doesn’t matter what kind of relationship you’re in. What I’ve found is that that people across the gender spectrum in conscious, sexual communities like polyamory, open-relationship, CNM, lifestyle, swinger, kink, BDSM, relationship anarchists, DADT, and any other flavor of group interaction all want to to enjoy worry-free and safe sexual adventures.

The Basics You Need To Know to Avoid STI’s

There are just a couple things you must know to be as safe as it gets:

  • Which tests to get (some labs sell ineffective tests, beware)
  • Incubation periods and testing windows
  • How often to test depending on your activities
  • When and HOW to have the “safe sex talk
  • What will really keep you safe (hint, it’s not just a condom)
  • And which sexual activities are relatively safe vs. much riskier. (You’d be surprised what falls into each category.)

As Easy As It Gets Easy To Protect The Sexual Health Of Your Group

This info really shows how important it is to get STI testing, and it’s also a reminder of how simple and private it can be to take care of your own sexual health, and your partners’ too:

The Harsh Reality

According to the CDC, half of all new STI cases occur in people aged 15-24, and untreated STIs can lead to severe health problems like infertility, increased risk for HIV, vaginal and urethral infections, and longer term issues such as neuralgias and cognitive issues. These statistics underline the critical need for regular and comprehensive testing.

Many in sexually active communities report the inconvenience and discomfort of traditional STI testing methods, which often leads to neglect and the spread of STIs. The core issue is not just getting tested, but doing so in a way that is comprehensive, convenient, and confidential.

Going For A Blood Draw Is Stressful And Unnecessary

Maybe you’ve experienced this too. You know you should get tested, but the thought of visiting a clinic makes you uncomfortable or anxious. Studies have shown that privacy concerns significantly deter individuals from getting timely STI tests, which can be crucial for their health.

Research suggests between 2%-10% of people who get faint or woozy from getting their blood drawn. Approximately 3% to 4% of the population experiences a significant fear of blood, known as blood-injection-injury (BII) phobia. An additional 15% feel uneasy or mildly triggered by the sight of blood.

You end up feeling embarrassed, discouraged, and overwhelmed. Sometimes it might even feel like despite your best intentions, you’re just not able to keep up with the necessary health checks.

But There’s A Reason Why

If you’ve ever felt this way, let me reassure you that you’re not alone. And it’s not your fault if you’ve found it challenging to maintain regular STI testing.

You see, the reason why it’s tough to stay on top of sexual health is that traditional methods assume everyone is comfortable with clinic visits, blood draws, and discussions about sexual health, which isn’t always the case.

Thinking that little annual testing of a couple of STI’s is “enough” or assuming that your partners are STI-free without verification are common pitfalls.

The Smell You Never Forget

You would NOT want to see what I’ve seen. Think a blood draw might make you woozy? How about pus-filled penises, giant lesions eating away at genitals, and the odor! My god the odor of STI’s gone rampant is a smell you never forget.

Assuming a condom will keep you safe is too risky. Did you know that the majority of STI’s are transmitted via skin-to-skin contact?

  • STIs transmitted via skin-to-skin contact:

Human Papillomavirus (HPV), Herpes Simplex Virus (HSV), and Syphilis are the most common STIs transmitted through skin-to-skin contact. These infections occur when skin, often in the genital, anal, or oral areas, comes into direct contact with infected areas.

– Skin-to-skin transmission accounts for a significant percentage of STIs, particularly for HPV and herpes, which are some of the most prevalent STIs worldwide. Estimates suggest that around 50-60% of sexually active people will contract HPV at some point, and the risk is predominantly through skin-to-skin contact.

  • STIs transmitted via bodily fluids:

Chlamydia, Gonorrhea, HIV, and Trichomoniasis are primarily transmitted through the exchange of semen, vaginal fluid, or other bodily fluids. These infections tend to spread through unprotected sexual contact (vaginal, anal, or oral sex) involving fluid exchange.

– The majority of these infections are transmitted via fluids, making up a substantial proportion of the total number of STIs. For example, HIV and Chlamydia are almost exclusively spread through the exchange of fluids rather than skin contact.

While exact percentages can vary depending on the population and the study, STIs spread via skin-to-skin contact are a significant contributor, especially for conditions like HPV and herpes. On the other hand, **fluid-based transmission** is dominant for bacterial and viral infections like chlamydia, gonorrhea, and HIV.

Answers In 24-48 Hours

But it’s not just getting the STI’s, it’s the long-term effects of going undetected. Dana knew. She heard about Alzheimer’s and neuropathies. We are barely scratching the surface of the negative contribution that having some of these viruses can have on our healthspan.

Being a physician-scientist with deep expertise in infectious diseases and sexual health, I’ve seen some gnarly stuff. That’s what drew me to create a next-generation technology company laser-focused on using the latest testing lab machinery with 21st century logistics to get YOU answers within 24-48 hours of receiving your tests. With an MD from Emory University and a PhD in Bioengineering from Georgia Tech, I’ve spent decades integrating technology with medicine to improve patient care.

Herpes Studies

There is a growing body of research that links Herpes Simplex Virus (HSV), particularly HSV-1, with cognitive dysfunction, neuropathy, and even Alzheimer’s disease.

  1. HSV and Alzheimer’s Disease: Several studies have suggested that HSV-1 may play a role in Alzheimer’s disease, especially in individuals who carry the APOE-ε4 allele. HSV-1 can remain latent in the body and become reactivated, leading to inflammation and other cellular damage in the brain. A study by *Itzhaki and colleagues* found that HSV-1 DNA is present in the brains of Alzheimer’s patients, particularly in regions like the hippocampus, which is critical for memory. Their research suggested that repeated activation of HSV-1 could lead to the accumulation of amyloid-beta plaques, a hallmark of Alzheimer’s disease.
  2. Cognitive Decline: HSV-1 has been linked to cognitive impairment in otherwise healthy adults. In a study published in the *Journal of Infectious Diseases* by *Tarter et al.*, it was found that individuals with high HSV-1 antibody levels were more likely to exhibit cognitive dysfunction. This supports the idea that chronic viral infections, like HSV-1, may contribute to neurodegenerative processes.
  3. Neuropathy: HSV has also been associated with neuropathy, particularly in the context of postherpetic neuralgia. HSV can infect nerve cells and cause chronic pain and sensory disturbances. This is seen in cases of herpes zoster (shingles), which is caused by the varicella-zoster virus (a form of herpesvirus) but is similar in its ability to affect the nervous system. Chronic HSV infections can lead to neural inflammation, potentially resulting in neuropathic pain.
  4. Cognitive Dysfunction and Immune Response: Chronic HSV infection can induce an inflammatory response in the brain, leading to cognitive decline. Studies like *Letenneur et al.* showed that individuals with HSV-1 were more prone to cognitive decline over time, particularly in functions like memory and executive functioning.

These findings suggest a multifactorial relationship between HSV infection and brain health, particularly in connection to chronic viral activation, inflammation, and immune responses. Further research continues to explore the mechanisms linking HSV to neurodegenerative diseases like Alzheimer’s.
